Seneca County Community Health Assessment and Community Health Improvement Plans

The Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) are two essential, interconnected phases of a collaborative process used by health departments and hospitals to improve a community’s well-being. Seneca County is one of the few counties in the state that does not have a hospital located within the county; therefore, the Seneca County Health Department collaborates with UR Medicine – Finger Lakes Health (Geneva General Hospital). 

What is a Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP)?

A CHA is the diagnostic phase. It is a systematic process of gathering and analyzing data to understand the current health status of a specific population. 

A CHIP is the strategic phase. It uses the findings from the CHA to create a long-term roadmap for action. 

The priorities of the 2025-2030 Seneca County CHIP are as follows:

  1. Healthy Children & Preventive Services
  2. Primary Prevention, Substance Misuse & Overdose Prevention
  3. Nutrition Security
